A+ Family Medicine would like to introduce you to a service we offer called Care Management. Care management can help improve the health of patients who have chronic diseases like diabetes, hypertension and/or heart disease or assist patients who are experiencing barriers in managing their health and everyday living. We want to offer this service to patients who might need extra help managing these health problems. This care management service allows you to work closely with an RN Care Manager. The care manager can assist you in figuring out ways to make changes to your health, ensure that all your care providers understand your needs, and help you deal with some of the stresses that may be interfering with getting better. Your primary care provider (PCP), may also recommend this service to you, although it is completely voluntary. This service is in addition to the care you receive from your PCP, who will still be your main provider.
