Your health insurance is a contract between you and your insurance company most likely setup by your employer. Your insurance representative, rather than our office staff, will be best able to answer your specific questions about covered benefits, etc. We do accept assignment from Medicare and participate with “traditional” Blue Cross. We are primary care providers with Blue Care Network, Priority Health, and various PPOs. To determine if we participate in the plan offered by your employer, consult the list of providers supplied by the particular plan, or look at the list provided below.

We will provide you with the proper information about the services rendered and the appropriate diagnosis, to enable you to correctly complete your insurance forms. Often, the encounter form you receive from the front desk as you check out will suffice when attached to your insurance claim form.
