Welcome to A+ Family Medicine!
New patients please make sure you have an insurance policy that lists the providers of A+ Family Medicine as participating providers. Here is a list of the insurance providers with which A+ Family Medicine currently does business.
Before your first appointment, the following items need to be addressed:
- If your insurance requires you to sign up with a primary care provider, call the insurance customer service number located on your card and register your new A+ Family Medicine provider. Make sure you complete this step before your first appointment.
- If you need to schedule an appointment with your new provider, you may call our office at (616)285-6450 or register on our patient portal (link) to request an appointment. Click here to learn about our scheduling policies.
- At your first appointment we will need you to complete several registration forms. You may also print these forms off, complete them at home and bring them to your first appointment:
- Patient Registration. This form gives A+ Family Medicine information about you and your insurance provider.
- A+ Family Medicine Office Policies. This document needs to be read and signed so you are aware of the patient policies of A+ Family Medicine.
- Health History form. This form will give us information about your current and past medical history.
- Consent for medication history. This form allows us to download your medication history into your medical record.
- Patient/Provider Compact. This form outlines your participation/responsibilities with regards to your care and the responsibility of your provider.
- Notice of Privacy Practices.
- Personal Representative. This form allows you to designate someone to speak to us on your behalf.
- Protected Health Information Release Form.
