With the increasing complexity of medical care, referrals to specialist physicians and other health care providers are frequently necessary. If you desire our staff to make or authorize a referral for you, this is the appropriate page. Referrals could either be new or in follow up of a prior encounter with a specialist. This referral could be upon the recommendation of your provider at A+ Family Medicine, or may be of your own initiation.
If you prefer, particularly in the instance of a return appointment with a specialist, you can make your own appointment and submit the details to us here for authorization. Alternatively, you can request that we set up the appointment for you.
In either case, please be aware that your provider may request to see you first if it is felt that more information is needed before a proper referral can be made, or if it is more appropriate for your complaint to be evaluated at this office first. We will notify you if this is the case.
Most insurance companies specify that authorization of non-emergent appointments be made two business days prior to the actual appointment and that post dating is not allowed. Preauthorization is not possible after an appointment has occurred and A+ Family Medicine will NOT backdate a referral for any reason.
If the situation is emergent, or if it is urgent and you are out of town, you do not need to contact us first, but are typically required to notify us within 48 hours.
